Attached please find a form (PDF) to use for students who will need to take prescription medicines while on the Chicago Trip.  It can be filled out and faxed to the school office (419-877-5028) or brought to school on Friday morning.

Students who need to take prescription medicine should turn in their meds to Mrs. Hoellrich with the form (unless it was faxed earlier) on Friday morning.  The medicine will be given to the students at the proper time based on the medicine forms.

Students who already have an over the counter medicine form on file in the office will be able to get OTC medicine based on that form.  If you have not turned in an OTC form to the office at the beginning of the school year and wish for your child to have access to those medicines, you may fill out an OTC Med form (PDF): available on-line under notes from the nurse.

We will also have hard copies of these forms available to your child at school this week.

The annual DVD video for the 2011‐2012 school year will soon be ready for distribution.

The multiple disk DVD set covers not only the marching season, including all parades, but also includes the concert season as well.

The DVD is only available on a “pre‐order” basis. If you are interested in obtaining one or more copies of the 2011‐2012 DVD, please fill out the information in the attached form (PDF) and return this form with the appropriate fee (cash or check made payable to AWBB).

WE NEED YOUR DESSERT DONATIONS for the May 12th Chicken BBQ.

All band students grades 5 through 12 are expected to bring desserts. For families with more than one band student, only one dessert is required. An assortment of desserts are needed. We are requesting your choice:

  • 1 large pie
  • 9 x 13 cake
  • 2 dozen cupcakes
  • 9 x 13 brownies or bars

Please have in a disposable container. No desserts that need refrigeration.

We need 1000 dessert servings!

We need every band family to fill out the form at the bottom of this flyer (PDF) and return it to their band director by Friday, May 4th. (Or send email to the address listed on the form.)

We realize that everyone is busy. If you are unable to make a dessert, please fill out the form at the bottom of this flyer (PDF) and return it with $10.

We will be taking the concert band pictures for the yearbook on Thursday, April 5 (the last day before break).  Students will need to bring their concert wear to school on Thursday.

This year we are offering picture packages for students who would like pictures in their concert outfits.  You are not required to purchase any pictures.

If you would like to purchase picture, a form is attached to this message (PDF).  Please bring money on picture day.

Please note: Students will not be taking section pictures; only a full band shot.  Students may choose to take individual pictures of themselves or someone else IN THE SAME BAND.  Students will not be able to take pictures with others in another band as the pictures will be taken during class.

Parents,

This is a form (PDF) that was sent home with the students on Friday (in case it didn’t make it home).  It is a waiver for the Kentucky Derby Parade.  Every participant (band member) must fill it out and return it to me by Tuesday to march in the parade.

Anthony Wayne Band Students and Parents,

As you may have already heard, the Marching Generals will travel to Louisville, Kentucky this Spring to participate in the Kentucky Derby Parade.  We will leave Anthony Wayne on Thursday, May 3rd, 2012, and return on Friday, May 4th.   Please note, this does not include the day of the race.

The trip will include the following:

  • Charter Coach Transportation
  • One night hotel accommodations
  • One Breakfast
  • One Dinner
  • Admission to the Kentucky Derby Museum
  • Admission to the Newport Aquarium

The cost of this trip will be approximately $220 per student.  It is possible that the cost of the trip could fluctuate as more details of the trip become evident and transportation costs are finalized.  This would affect the final payment.

To secure a spot on the trip a deposit of $50 will need to be paid by September 30th.  The entire payment schedule is as follows:

  • September 30th: $50 Deposit
  • December 1st: $75 2nd Payment
  • March 15th: Balance

Band fans who would like to travel with the band will be accepted on a space available basis in the order that their deposit is received.  The cost for the trip with double occupancy will be $258 per person.

Please return the bottom of the attached form (PDF) with your deposit by September 30th.

Checks should be made out to AWBB.
